The information below serves as a guide on what to expect after scheduling your initial consultation, along with the next steps. Please reach out with any questions or concerns.

New Patient Paperwork

After booking your initial consultation, you will receive an email with intake and consent forms. To ensure we have ample time to review important details before your appointment, we kindly request that all forms be completed within 24 business hours of receipt. If forms are not submitted within this timeframe, your appointment may need to be rescheduled.

Appointment Info

You will receive reminders with your appointment date and time, along with a link to join our secure, HIPAA-compliant video session. All appointments are completed virtually/telehealth via Zoom at this time. We recommend logging in 5-10 minutes early to test your connection and technology. Please be prepared to provide vital signs, including blood pressure, heart rate, temperature, and your current height and weight, during your visit. At your appointment time, you will be automatically connected to our video session.

Initial Consultation

Your first visit will be a consultation for the purpose of evaluation. After evaluation, we will offer some first impressions of what our work might include, a treatment plan, and considerations regarding psychotherapy, medications, and holistic approaches, as indicated. Together, we will formulate a plan to address your concerns and improve your mental health.

Patient Communication

To communicate securely with the practice, please download the Spruce app on your phone. This ensures your medical information remains private while enabling easy communication—simply send a text through the app. We typically respond within a few hours or by the next business day

Booking Appointments

The booking link allows you to schedule an appointment. While follow-up appointments are typically arranged during your visit, you can use this link if an appointment hasn’t been scheduled or if you need to cancel and reschedule. Please select whether you are a private pay or insurance pay patient, and then choose the appropriate follow-up appointment category.

Patient Portal

A patient portal is a secure online website that can help you access medical records, set up appointments, etc. You may receive documents and other important information from the portal, but please note that our communication platform is Spruce and the messaging feature in the portal has been disabled.
